Disability

We recommend that you fill the direct deposit request so that you benefit payments are deposited directly into your bank account. Just download the form, fill it and send it to us.

Depending on the type of claim, the number of forms to fill out or to be completed by an intermediate (doctor, employer, etc.) can vary. If in doubt, contact your employer or our customer service.

• Complete the Disability claim form
• Fill the Physician's statement that applies to your medical condition, either the Physical illnesses form OR the Psychological illnesses form
• Complete the Standard authorization for disclosure of information

If you have been insured for less than a year, complete the authorization for disclosure of information specific to your province:

• Authorization for disclosure of information - Alberta
• Authorization for disclosure of information (British Columbia)
• Authorization for disclosure of information (New Brunswick)
• Authorization for disclosure of information (Nova Scotia)
• Authorization for disclosure of information - Quebec (French only)
• Authorization for the disclosure of a driving record by the SAAQ (if pertinent to the claim)
• Authorization for disclosure of information- Ontario

Additional reports to fill during your claim's life only upon demand:

• Physical illness
• Psychological illness
• Notice of return to work
• Employer's confirmation - Gradual return to work (Document to be completed by the employer in case of a gradual return to work)

Medical, paramedical and dental insurance

Drugs

Receipts must indicate the patient’s name, drug name and drug identification number (DIN).

Medical and paramedical fees

Receipts must indicate the name and address of the specialist and the date of each visit or examination as well as the related costs. Consult your booklet to know your protection for professional care (chiropractor, physiotherapist and others). Please attach a medical referral when required by your contract.

Vision care

Receipts must indicate the name and address of the specialist and separately, the costs of contact lenses, prescription eyeglasses, frames, the exam and its date.

Dental care

Some treatments require a physician's statement and therefore cannot be claimed online: Dental care following an accident, removable dental prosthesis, fixed bridgework, crowns, veneers, inlays.
